Understanding treatment-resistant depression
What is treatment-resistant depression? TRD is a form of major depressive disorder (MDD) that doesn’t respond well to typical interventions like antidepressants or therapy. In clinical terms, it’s often defined as depression that continues after trying at least two different antidepressant medications at adequate doses for a sufficient length of time. The U.S. Food and Drug Administration (FDA) and the European Medicines Agency (EMA) have adopted this definition of TRD.1
Nearly one-third of those actively seeking help for depression have TRD—proof that not finding relief right away doesn’t mean you’ve failed or that healing is out of reach. TRD can be isolating and frustrating, especially when your efforts to feel better don’t yield the results you’re hoping for.1 But the good news is that newer and more effective treatments, such as Spravato® (esketamine), are expanding the possibilities for recovery.2
What makes depression “treatment-resistant”?
Major depressive disorder shows up in many different ways. Some people experience symptoms that are primarily emotional—persistent sadness, guilt, or loss of interest in life—while others may notice physical changes, like disrupted sleep or appetite.
TRD is not a separate condition from depression but rather a way of describing how your symptoms are responding—or not responding—to current treatment approaches. Factors that may contribute to TRD include:
- Genetic or biological differences in how your body metabolizes medication
- Co-occurring mental health conditions like anxiety or bipolar disorder
- Undiagnosed physical health issues that impact mood
- External stressors or trauma that haven’t been fully addressed
Your mental health provider can help identify any underlying issues that may be contributing to your current struggles and create a tailored plan to move forward.
What are the symptoms of TRD?
The symptoms of TRD are often the same as typical depression, but they continue despite trying more than one standard depression treatment. You might be experiencing:
- A persistently low or flat mood
- A sense of hopelessness or emptiness
- Trouble concentrating or making decisions
- Fatigue that doesn’t go away with rest
- Difficulty sleeping or sleeping too much
- Ongoing thoughts of worthlessness or self-blame
It’s also common for people with TRD to feel emotionally numb or disconnected from the world around them. These symptoms may fluctuate but rarely go away entirely.
Exploring options beyond traditional antidepressants
At LightHeart Mental Health, we’re always available to give our clients the help they require at any time and place. If you’re not finding relief with standard antidepressants, there are other depression treatment options to explore, including:
- Spravato® treatment: Spravato® (esketamine) is an FDA-approved nasal spray medication for adults with TRD. It works differently from typical antidepressants by targeting N-methyl-D-aspartate (NMDA) receptors in the brain. Unlike many medications that can take weeks to become effective, some clients experience noticeable improvements within hours or days.
- Therapy with a specialized focus: Cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT) are often effective, especially when combined with other interventions. Our therapists offer a range of modalities tailored to each person’s needs because treatment is never one-size-fits-all.
- Psychiatric medication management: Our psychiatric providers work with you to refine your treatment, whether by adjusting the dosage, switching medications, or considering combination approaches.
- Group therapy and support networks: Finding a space to connect with others who share your experience can reduce isolation and help you build community and perspective.
We provide essential in-person mental health care services and offer telehealth options for added flexibility.
Frequently asked questions (FAQs) about TRD
What causes treatment-resistant depression?
There’s no single cause. It may be related to biological differences, chronic stress, early trauma, or even the specific type of depression you’re experiencing. Some people may also have genetic markers that impact how they respond to certain medications.
Is treatment-resistant depression permanent?
No. With the right combination of therapies, medications, and support, many people do experience significant relief—even after long periods of difficulty. TRD is a challenge, not a dead end.
Can Spravato® work for you?
Spravato® may be appropriate if you’ve tried at least two antidepressants without meaningful improvement. It’s administered in-office under supervision to monitor for side effects and ensure a safe experience. Clinical trials show it can reduce symptoms rapidly for many people with TRD.
What types of depression are more likely to be treatment-resistant?
Persistent depressive disorder (PDD), bipolar depression, or depression accompanied by anxiety or PTSD can sometimes be harder to treat. That’s why it’s so important to receive a thorough evaluation from a qualified mental health provider.
